If it is your first visit, please visit our New Patient Forms section and print to fill out your forms before arrival, or come 15 minutes early to fill them out at the office. Cancellation policy: Cancel by contacting Lisa directly via email or phone/text. If an appointment is missed without 24 hour notice, you may be charged 50% of the normal visit. |
